Your health is important to us. Neighborhood has a number of clinical programs and resources to help members manage chronic conditions and complex health care issues in order to live a healthy life. Asthma: Breathe Easy

Breathe Easy includes educational materials to help members with persistent asthma live active, healthy lives. Asthma can make breathing difficult. In this program, you will get information on how to manage your asthma better.

Chronic Obstructive Pulmonary Disease: Take a Breath

Take a Breath includes educational materials to help members with COPD live active, healthy lives. COPD is a lung disease that keeps your lungs from working normally. This program will teach you how to manage your COPD better.

Coronary Artery Disease: Keep the Beat

Keep the Beat includes educational materials to help members with coronary artery disease (CAD) live active, healthy lives. Having CAD raises your risk for a heart attack and other problems. This program will teach you how to manage your CAD better.

Diabetes: Control for Life

Control for Life includes educational materials to help members with diabetes live active, healthy lives. Diabetes can lead to excess sugar in your blood. This program will teach you how to manage your diabetes better.

Heart Failure: Healthy Heart

Healthy Heart includes educational materials to help members with Heart Failure (HF) live active, healthy lives. HF is a condition where the heart cannot pump the amount of blood your body needs. This program will teach you how to manage your HF better.

Healthy Pregnancy Program

Bright Start is Neighborhood’s pregnancy program to helps members have a healthy pregnancy and a healthy baby. Click here to learn more about the Bright Start program.

Get Care with Health@Home

Our goal is to help you be as well as you can be. Health@Home* is for members with chronic conditions who need extra care. Members who participate in Health@Home need a referral from a provider since the program is an extension of your primary care provider’s treatment.  Members in the Health@Home program get care from licensed nurse practitioners during home visits and by phone.

Neighborhood’s Be Well, Stay Well newsletter is made for members with chronic conditions, including asthma, Chronic Obstructive Pulmonary Disease (COPD), diabetes, heart failure, and coronary artery disease (CAD).
