PREMIER Elite, PEAK Elite, and PRIME Elite Point-of-Service (POS) plans offer exclusive benefits. Elite plan members:
- Can go out of network for certain care and services
- Will have a separate out of network deductible, out of pocket maximum, and cost sharing
- Will be required to have an in-network Primary Care Provider
- Are still required to get prior authorization for all services that require a prior authorization in network
