Claim Submission Reminder
Neighborhood uses technology to scan paper forms and eliminate keystroke errors. All claims must therefore be submitted on original (not photocopied) print versions of the industry standard CMS-1500 and CMS-1450 (UB-04) forms, as they are printed in special optical character recognition (OCR)-scannable red ink.
Copies and faxes are not accepted, and must not contain any writing or Wite-Out®.
New Document: Form Finder
A guide has been added to the Claim Forms section of the Provider Manual Forms menu on the Neighborhood website. This document lists several different scenarios and can help to determine which form must be completed and submitted to Neighborhood, along with additional information related to the submission of each form.
Updated Forms
Updated provider request forms are available in the Claim Forms section of the Provider Manual Forms menu on the Neighborhood website. The Corrected Claim, Reconsideration, Adjustment, and Appeal Request forms have been reformatted. In addition, all four of these forms are now writeable, so the information can be typed into the document, then printed and submitted to Neighborhood.
Updated Policies
Neighborhood’s policy on Urine Toxicology Screening has been updated to follow the industry standard practice of using panel codes in place of individual drug testing codes. The Reason Codes listing has also been updated.
