Connect CARRE

The Connect CARRE program can help to improve your health and wellness if you are at risk due to complicated medical issues. This program is for Medicaid recipients age 22 and over who meet certain eligibility criteria.

How can we help you and your family?

Can help you to find a medical doctor or specialists
Can provide a list of available transportation to help you to get to doctor’s appointments
Can assist in finding community resources such as food pantries
Can provide resources to assist in obtaining adequate housing
Can help you to better understand your medical conditions
Can help to obtain needed home services or equipment

Referrals to the Connect CARRE program can be made by contacting the Program Manager, Susan LaSalle, RN, at 459-6644.
