Substitute Care Program

The Substitute Care Program can help improve the health and wellness of children in foster care.

How can we help you and your family?

  • Can help arrange the care the foster child needs
  • Can help to refer to programs for medical concerns with a Registered Nurse
  • Can help coordinate care between multiple providers
  • Can help coordinate care between biologic parents, foster care parents and DCYF Case Workers
  • Can help refer to programs for behavioral or emotional concerns.
