The Pediatric Care Management program can help to improve your health and wellness if you are at risk due to complex medical and/or social issues. This program is for members under the age of 19.

How can we help you and your family?

Facilitation and coordination of:
- Transportation to doctor appointments
- Refer you to programs if you are depressed
- Help you make doctor appointments
- Set-up medical equipment
- Set-up any services you need at home

Address social concerns:
- Domestic violence
- Housing
- Food
- Community resources
