At Neighborhood, we aim to: Keep members well through health care check-ups Help members learn healthy habits Get members good care when pregnant Work with members to manage their chronic conditions such as asthma, chronic obstructive pulmonary disease (COPD), heart failure (HF), coronary artery disease (CAD), or diabetes Help members in finding resources and information they need to make good health care choices Nurses and health educators can help members with the following conditions: Neighborhood provides Care Coordination and Case Management services for individuals with special health care needs. Our Social Care Coordinators and Nurse Case Managers follow your case as you go through the health care system. Some illnesses are very complex and frightening. They may involve tests, surgery, treatment or hospitalization. The purpose of a Care Coordinator/Case Manager is to have one person be your point of contact to help you.
