Attention Deficit Hyperactivity Disorder (ADHD)
Neighborhood's program is designed for members age five to eighteen having ADHD

The goals of the Neighborhood ADHD Program are to:
• Aid providers in properly diagnosing and managing ADHD
• Monitor and improve the health of children having ADHD
• Support parents of children having ADHD
• Support schools in diagnosing and treating children having ADHD

ADHD Tool Kit: This tool kit is intended primarily for physicians but it may also answer teacher and parent questions about ADHD as well. The tool kit has forms regarding evaluation, comorbidity assessment, follow-up visits, and billing, as well as rating scales for parents and teachers to help find out if a child has ADHD, tips for teachers, and a medication fact sheet for parents and teachers.

Goal-setting Guide for Doctors and Parents – This page from the American Academy of Pediatrics (AAP) contains a form to help doctors, parents, and teachers create a daily school/home report card for the child with ADHD.
